Acceptable documentation for your ImPACT Baseline Test clearance
Steps for Completing the ImPact Baseline Test, through Ankored.
- Login to your Ankored Account
- View your requirements and select "Complete Online" for the ImPact Baseline Test Requirement.

ImPact Baseline Test Steps:
- Select "Buy Baseline Test"
- You will be prompted to fill out the screen below:
- Fill out the necessary information, along with your credit card information, and select BUY TEST.

Timeline: The test should take no longer than 45 minutes to complete.
Once the test is completed, please download your results, navigate back to Ankored, and upload them for approval:
ImPACT Baseline Test Documentation Requirements
To meet the requirements for ImPACT Baseline Testing, the test type must result in: Baseline.
In addition to confirming the test result, it is necessary to upload one of the following as proof of completion in Ankored for Admin review:

Examples of Acceptable Documentation:

Completion Certificate:
Includes the participant's name, date of birth, test date/time, test type (Baseline), and Confirmation ID. -
Completion Email:
A confirmation email from ImPACT Applications verifying test completion, including the Passport ID. -
Clinical Report:
Official document from ImPACT Applications showing the participant’s name, Passport ID, date of birth, organization, and gender.
Please ensure submitted documentation clearly shows the Baseline result and includes one of the three listed proof types.
Results:
- Once you have uploaded your results your Test status will show as "Pending Review"
- An Ankored Admin will review your Test and you will receive an email letting you know if it is "Approved" or "Not Accepted."
- If "Approved", nothing else is needed. Your status will go from Pending Review to Completed. If this is your last requirement, your overall compliance status will move from Incomplete to Cleared. This Cleared status is directly synced into LeagueApps.
- If "Not Accepted" there will be information explaining why (such as "Incorrect form") and you will have the option to try again, or email Ankored Support: support@ankored.com
